An environment is a stage: the ground, the hazards, the enemy pool and the interactables a run drops you onto. There are 31 of them, and a single run only sees a fraction, because the sequence is assembled as you go rather than played from a fixed list.
The run itself is built on one repeating event: you find the teleporter, hold the area while it charges, and move on. That event is what advances the run, and it is also where the difficulty curve catches up — the longer you spend on a stage, the more you carry into the next one and the harder the next one is.
How shrines and secrets actually appear
This is the part most environment pages get wrong. Shrines, altars, chests and hidden nooks are not placed at authored coordinates — they are placed by the stage generator inside a layout, and some environments ship more than one layout for it to choose from.
